Letters for Parents

Please note:

We have included some of more frequent requests/general letters on this page to allow you to read them/fill them in at home before passing them into the office or sending them into your child's class teacher. If you are looking for copies of the most recent letters we have sent home, please click on the Letters Home option from the Parents Menu above.

Home Emergency Contacts form

This is our standard change of address, telephone/mobile phone form. It is really important that we have your most current details on our school records at all times, in case of emergency. It is now a requirement by law, that we have the contact details of two parents/guardians on record.

Photography Consent Form

This is our standard parental consent form for photography within school. It is important that we keep a record of parents who wish/do not wish their children to have their photographs taken for use within school (e.g. school trips, sports days, performances...)

Medical Review form

This is our standard medical form. Annually, we will ask you to update the information on your child's medical record form, to ensure that we are aware of any changes to your child's health. If you wish to inform us of any changes between times, please complete this form and pass it into the office or send it in with your child.
